Description
Summary
The Physician Assistant (PA) practices in Home Based Primary Care Service (HBPC) to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The PA provides advanced clinical practice - consultation - and management. NOTE: The 2-page Resume requirement does not apply to this position. For more information - refer to Required Documents below.
Duties
- VA offers a comprehensive total rewards package
- Duties include but not limited to: The Physician Assistant (PA) is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients
- Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice in the patient's home
- Develops and implements an individualized plan to achieve optimal population health and disease management
- Demonstrates leadership and works collaboratively with Home Based Primary Care (HBPC) interdisciplinary team members
- Orders and interprets labs - tests - and diagnostic studies
- Prescribes pharmacologic and non-pharmacologic interventions
- Initiates referrals and consults to specialty services
- Provides education to patients and their families
- Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
- Determines the need for and facilitating patient admission to the VA medical facility or other transitions of care

- Work Schedule: Monday-Friday - 8:00am-4:30pm Compressed/Flexible: May be available and will be discussed upon selection Telework: Available on an as needed (Ad-Hoc) basis as determined by the supervisor Virtual: This is not a virtual position
- Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Contract Buy Out Program: This position may be eligible for the Physician / Provider Contract Buy Out Program (CBOP) - which authorizes VHA to buy out service contracts (Locum agreements - loan repayments - sign-on bonus contracts - Physician-owned practices) in exchange for employment at certain rural or highly rural facilities for a period of obligated service (minimum 4 years)
